I think that: I think the lottery should be operated by the government ie civil servants, and all winners should have a the specified amount won each week.
Because: A more regulated approach would let people know exactly how much they would win for their numbers and there would be no fat cats creaming off the profits thus more money would go to the right causes, mainly the NHS, which could pay for the fat cat managers instead, but hopefully more nurses and better patient care as a result.
