The real issue with transporting children on bicycles is NOT getting 
their feet in the spokes. Otherwise they bounce pretty good ;)

Which is why Dutch bikes have skirt guards... so your pal sat on the
rear rack doesn't dangle their feet into the back wheel by mistake.

Shouldn't the child have his own bike at 4 years old, or be walking?  You
have to be pedalling on the bike for it to keep you fit and healthy.

I was more interested (to the extent I was at all interested, IYSWIM) in

I would say, from having done that sort of "stupid" thing myself (from
the kid's perspective, not the parent's) for the last 40 odd years, that
the kid had /himself/ balanced with his mother's co-operation.  Put a
child somewhere they don't feel safe and they will say so and make
attempts to change matters.
