I witnessed a class 25 do that at Guide Bridge years ago, spilt
ballast got in the point blades and down it went all axles, tremendous
crash , rerailed within a couple of hours though.

It does seem that the loco was moving at 12mph at time of derailment.
BRUFF (Road/Rail Vehicle) on site since 1630.
