journeyplanner.tfl.gov.uk suggests 23:47, change at Victoria.
(YMMV :-)

Last train is the 0018 Victoria line to Victoria arriving at 0034 then
change for the 0040 District line to Hammersmith. If this seems a little
tight you may wish to aim for the 0007 from Finsbury Park instead.

.... though I understand that LU normally ensure that last train
connections are made, holding up the last District if the last Victoria
is delayed for example.
If the Piccadilly Line has been fully reopened by then (which is
probably unlikely), the last one would be the 00:16 direct to
Hammersmith.
Incidentally, planned engineering works for that weekend should not
affect the above times.

Don't run if you've got a coat on, carrying a bag, or look a bit
foreign either.

Following the events of the 7th of July, I'd be inclined to say there
isn't one.
Your best route would be Victoria Line to Victoria, then District Line
to Hammersmith.
Last Southbound Victoria Line train is at 0018 from Finsbury Park, due
Victoria at 0034. There's then an 0040 from Victoria, due into
Hammesmith at 0054.
In the normal course of events, the last Southbound Piccadilly Line
train would be at 0016, due Hammersmith 0048.

First & last trains at
http://www.tfl.gov.uk/tube/travelinfo/firstandlast/default.asp but of course
there may be changes because of the current disruption.
