What caused the 'Thames Whale's' scar? Was it 'guided'?

The whale that was in the River Thames in central London yesterday was
reported as having a 'scar' or 'growth' on the left side of its head.
What caused this? Has someone got a photo?
I wonder whether the unfortunate animal hadn't been operated upon
